MedCore Talent has been selected by a well respected, progressive healthcare organization to screen and recruit the ideal Transitional Care Registered Nurse to serve on theirBundled Payment for Care Improvement program (BPCI) team. This position is a full-time, facility based, permanent position with a VERY competitive salary and great benefits.
The Transitional Care Nurse is responsible for overseeing and facilitating all transitional care activities under the organization's Bundled Payment for Care Improvement program (BPCI) from initial hospital admission to the conclusion of the BPCI period, typically 90 days post-discharge. Transitional care activities include patient assessment and education, discharge planning, development of a post-acute care plan, arrangement and coordination of post-acute services, and direct follow-up on patients progress relative to their post-acute plan. Success in the role will be defined by reductions in avoidable patient complications and adverse events, and prevention of wasteful post-acute resource utilization. The position reports to the Market Leader with dotted line reporting to the VP of Transitional Care.
Responsibilities Include:
Prospective identification and confirmation of bundled care patients.
Patient assessment to determine the appropriate level of postacute care and mitigate readmission risk.
Development of inpatient and transitional care plan in collaboration with physicians and other stakeholders.
Patient and family education to ensure understanding of transitional care plan and ongoing oversight of that plan, including transfering clinical information between providers, scheduling of transitional care services, facilitation of key clinical information transfer between acute and postacute providers, facilitation of care management meetings, and ongoing review and reporting until discharge.
Review of BPCI process and outcomes performance to drive performance.

Job Requirements:

BSN or CCM required;
Minimum of Five (5) years of acute care nursing experience.
Two (2) plus years of experience in case management, transitional care, or post-acute services.
Ability to work independently, multitask and prioritize workload in a fast-paced hospital/clinical environment.
Excellent organizational abilities with an attention to detail and accuracy.
A driver of processes with strong interpersonal skills and equipped to handle sensitive situations and confidential information while continually demonstrating poise, tact, diplomacy, and good judgment.
Ability to interact and communicate with individuals at all levels of organization, including and not limited to, a proven track record in collaborating effectively with physicians.
Client service oriented (both internal and external).
Positive attitude with an ability to influence providers and patients alike.
Proficiency with Microsoft Office Suite, ability to analyze patient data and high level of comfort with reporting data to key stakeholders and leaders.
